2025 was a year to remember for Primary Care 24!
From tackling health inequalities, to transforming access to care for some of our most vulnerable communities, the dedication of our colleagues was recognised on a national stage. Over the past year, we were proud to receive multiple awards and nominations that highlight the impact we are making and the standards we strive to deliver every day.
These projects were led by multidisciplinary teams across PC24, including our frontline clinicians and community partners, who all work together to improve care where it’s needed most.
UHUK Awards
UHUK is a partnership of urgent healthcare social enterprise providers, who cover two thirds of the UK population. Annually, they host an educational conference and an awards ceremony. In 2025, we were thrilled to have won four awards – one Gold Award and three Silver Awards!
1: Gold award for Equity in Healthcare (Health Inequalities Award)
With the Liverpool School of Tropical Medicine, we launched a health initiative to tackle antimicrobial resistance (AMR) and improve healthcare access for asylum seekers. With the help of academics, clinicians and creatives, we created accessible printed and electronic materials which explains how to access and use medicines in the UK.
This work has helped asylum seekers better understand how to access care and use medicines safely, reducing barriers, confusion and risk.
2: Silver Award for Making a Difference (Social Impact Award)
From 2024, we have delivered a range of initiatives to help reduce health inequalities across vulnerable groups, including using a Dentaid Bus to deliver vital dental care, on-site visits by the Sexual Health Bus, partnerships with foodbanks, annual Eid celebrations, clothes donation drives and other initiatives.
3: Silver Award for Stronger Together (Partnership Award)
After experiencing pressures on Out of Hours services, we partnered with the Urgent Community Response (UCR) team at MerseyCare. Our clinicians were experiencing cases that needed a UCR intervention, and we created a joined-up system to enhance patient care.
4: Silver Award for Outstanding Engagement & Involvement (Patient Engagement Award)
We identified a key gap in accessible, patient-facing information for asylum seekers, and developed accessible resources including a multi-lingual healthcare navigation leaflet, which is now distributed it at key touchpoints across the city.

The Nursing Times Awards 2025
We were proud to be shortlisted for three national awards at the Nursing Times Awards 2025, held in October. These prestigious awards celebrate outstanding leadership in healthcare.
We were shortlisted in the following categories:
- Nursing in Primary Care Award
- Patient Safety Improvement Award
- Nursing in the Community Award
We were delighted to take home two wins on the night, ‘Nursing in Primary Care’ and ‘Nursing in the Community’, recognising a project that is transforming access to healthcare for homeless and vulnerable patients in Sefton.
These awards celebrate the exceptional work of Hannah, our Head of Nursing in Primary Care, Dr. Ellis, our Clinical Lead for Primary Care, and their wider team, in partnership with Light for Life. Together, they have delivered innovative care across some of the most deprived communities in Merseyside, ensuring healthcare is accessible and tailored to individual needs.
North West Coast Research & Innovation Awards 2025
PC24 attended the prestigious North West Coast Research and Innovation Awards in April 2025, after being shortlisted as a finalist in the ‘Capacity Building in Research’ category.
The competition was incredibly strong, with many inspirational finalists across all ten categories. Being shortlisted was a fantastic achievement, especially as PC24 had been involved in research for just under 12 months at the time. To have come this far in such a short time is a testament to the passion and hard work of everyone involved.
BAME Health & Care Awards 2025
Two of our nurses, Clare and Hannah, were shortlisted for the BAME Health & Care Awards and Hannah took the award home!
Hannah (Head of Nursing for Primary Care) won the Nurse of the Year Award.
Hannah has lead transformative work in asylum and homeless health and co-produced inclusive safeguarding models with Light for Life. Hannah has driven innovation by redesigning asylum health screenings and creating the PC24 Safeguarding Champion Model, as well as embedding Protected Learning Time for nurses and mentoring the next generation of B.A.M.E leaders.
Clare (Practice Nurse) was shortlisted for the Advocacy and Patient Experience Champion Award
Clare has transformed the patient experience for our service users. By introducing an on-site phlebotomy clinic, Clare reduced waiting times from three to four weeks to just one week, ensuring timely care and better outcomes. Clare acts as an advocate for patients facing language barriers, trauma and systemic challenges – always treating each person with dignity and respect.
Nursing Times Workforce Awards 2025
In 2025, we were shortlisted for ‘Best Social Responsibility Programme: Health, Humanity and Hope: A Workforce-Led Revolution in Social Responsibility’ at the Nursing Times Workforce Awards, hosted in November. These awards celebrate innovation and excellence in workforce planning.
Khadija, our Head of Community Services, and the Community Team have developed an extensive social responsibility programme, focused on removing healthcare barriers for asylum seekers and homeless individuals. Through mobile outreach, community-led events, and micro-initiatives, the team regularly deliver urgent dental and sexual health services, support new mothers in hardship, and promote cultural inclusion through large-scale Eid celebrations.
Our 2025 Colleague Awards
Here at PC24, we have our very own Colleague Awards every year where we celebrate our amazing colleagues across all our services. There are 10 categories for our colleagues to nominate others, including Rising Star, Leader of the Year and Newcomer of the Year. On the awards evening, the winner of each category is given a trophy of recognition, along with a gift. This evening is a special occasion where we celebrate and give back to our colleagues for their hard work and dedication over the past year.
Conclusion
We would like to thank our PC24 team, who made these awards and nominations possible. 2025 was a great year, full of worthwhile projects, and we are looking forward to celebrating even more in 2026.
