What we do

We deliver NHS Primary Care services covering a population of 1.6 million across Cheshire and Merseyside.

How we’re structured

Each of our three services cover various locations across Cheshire and Merseyside, and each service is run by a Head of Service.

The model is split into the following three areas:

  • Primary Care
  • Integrated Urgent Care
  • Community Services

Primary Care Services

We offer a wide range of Primary Care services operating during daytime hours across Sefton, Knowsley, Liverpool and Halton. These include:

PC24 provides primary care services to a combined patient population across our 8 GP practices in Sefton and Liverpool.

Our Primary Care Hub is based in Litherland, Sefton, and provides care navigation for all our GP Practice patients.

We provide dedicated nursing care across two nursing care homes in Liverpool, supporting a total of 120 beds. Our experienced team ensures high-quality, compassionate care tailored to the individual needs of residents, contributing to safe, well-staffed environments where health and dignity are prioritised.

Integrated Urgent Care

Providing a number of Urgent Care services throughout Halton, Liverpool Knowsley, St Helens, Sefton, Southport, Formby and Warrington.

Liverpool, Halton, Knowsley, Warrington, Sefton and part of St Helens.
This service operates when GP surgeries are closed, providing telephone triage and face-to-face care for patients with urgent medical needs which cannot wait until their GP practice re-opens. Patients who require urgent medical attention outside of their usual GP opening hours can telephone NHS 111 who will undertake a basic assessment of their clinical need and transfer appropriate cases to PC24 for clinical assessment.

Liverpool
We provide Extended Primary Care Access services to around 750,000 patients, supporting local GP practices to meet the increasing demand on primary care. We provide additional GP and nurse appointments 7 days a week.

Liverpool, Halton, Knowsley, Warrington, Sefton and St Helens.
Alongside our colleagues at NHS 111, we support the provision of the Clinical Advice Service 24 hours a day, seven days a week. Patients who contact NHS 111 with pre-defined conditions, who would usually be referred to an Emergency Department (ED), are now referred to us for a further assessment by one of our team.

We support a local EDs in the Liverpool area – one of which is the paediatric streaming service at Alder Hey Children’s Hospital – by providing additional provisions for patients who present at ED with primary care needs, helping to relieve the overall pressures faced by hospital services.

The Paediatric Primary Care Streaming Service at Alder Hey Children’s Hospital is designed to streamline access to healthcare for children and young people, ensuring they receive timely, appropriate care based on their needs. This service directs patients to the best-suited healthcare professional for their specific concerns, reducing waiting times and improving the overall efficiency of care delivery.

We work closely with NWAS to provide GP telephone support for paramedics who are treating patients in the community. This service is available to NWAS paramedics 24 hours a day, 7 days a week.

We deliver an Outbreak Management Service that provides rapid, safe, and mobile vaccination and clinical response to infectious disease outbreaks across Cheshire and Merseyside. The service ensures timely vaccination of identified contacts in both in-hours and out-of-hours settings, maintaining strict infection control and cold chain standards. This flexible, clinically led response model supports the UK Health Security Agency in containing outbreaks and protecting public health.

This is a dedicated Paediatric 111 Clinical Advisory Service for the whole of Cheshire and Merseyside, delivering urgent, paediatric specific clinical advice to families with children aged 0-5 years. This service enhances the existing NHS 111 pathway, improving clinical outcomes and reducing unnecessary Emergency Department (ED) visits. The service offers fast, expert clinical guidance to families of young children, thereby avoiding unnecessary travel, anxiety, and exposure to infection. The service ensures consistent care across the Cheshire and Merseyside geographical region, with local residents able to access the service. This contributes to reduced health inequalities and better long-term health outcomes.

Community Services

Providing several Community Services across Merseyside.

The Asylum Seekers Initial Assessment Centre (IAC) service, is on behalf of NHS England, providing comprehensive initial health assessments for individuals arriving at asylum accommodation centres. The service ensures timely identification of physical and mental health needs, vaccination status, communicable diseases, and safeguarding concerns, with appropriate onward referrals and treatment. Operating Monday to Friday, it plays a key role in promoting equitable access to NHS care for vulnerable asylum seekers.

We provide GP support to patients admitted to intermediate care beds at four nursing homes, two in Knowsley and two in Southport and Formby. Our clinicians conduct thorough assessments of these patients and collaborate closely with the nursing home’s Multi-Disciplinary Team (MDT). This ensures each patient receives the most appropriate care and support tailored to their individual needs.
