Learning from Incidents
PC24 recently led on a Multi-Agency review of a case which raised concerns about the care that is sometimes delivered to people with Learning Disability.
Along with colleagues in the North West Ambulance Service, Alternative Futures and Aintree University Hospital, we conducted a Serious Incident Review and shared the learning from this at a learning event here at PC24.
We continue to monitor reported incidents and complaints we receive carefully and seek to work closely with other organisations to maximise learning for all.
PC24 assesses the clinical decision making and practice of it’s clinicians using Clinical Guardian software.
Developed by our colleagues in BrisDoc, it allows a random sample of a clinicians workload to be pulled and placed into a file for peer review.
